docs: audit plan/todo docs against current repo state #15

Summary

replace the stale greenfield implementation plan with an audited current-state summary
replace the stale backfill TODO proposal with an audited status/follow-up document
update the README pointer so plan/todo docs reflect their new purpose

Why

The repo had drifted away from llm/plan.md and llm/todo.md. Those files still described future work that is now largely implemented, which was causing issue and documentation confusion during planning.

Notes

this is a docs cleanup / repo-audit PR only
no runtime behavior changes
follow-up issues were also cleaned up separately during the audit pass
