Implements issue #32 by adding explicit many-to-many associations between raw thoughts and curated learnings, with MCP tools to link, unlink, and retrieve in both directions.\n\nVerification:\n- go test ./internal/tools ./internal/mcpserver ./internal/store\n- make build && make test
