Summary: added and linked format usage documentation, with DBML-to-writer examples, file-based reader examples, cross-format/live-database examples, and guidance for formats outside convert. Clarified the documented coverage and known GORM-to-DBML issue.

Verification:

git diff --check — passed.
go test ./... — passed.
Built the CLI and exercised the documented DBML-to-writer examples for JSON, YAML, DCTX, DrawDB, GraphQL, Prisma, PostgreSQL, MSSQL, SQLite, Drizzle, TypeORM, GORM, and Bun — passed.
Remote branch readback matches SHA above.
